COVID-19 Protocol

As we have all been dealing with the effects of the pandemic, there are guidelines that we need to have in place for the safety of all people involved. The doctor will not be able to enter the premises without all persons present wearing an appropriate face mask covering both nose and mouth. We understand that this is a very difficult time, and tears will be shed and tissues used; however, our doctors need to maintain safety from spreading illness to other homes and contracting the illness themselves. If you do not wish to wear a mask at this time, Tranquil Crossings will not be able to assist you in your home. The doctor will need to be in a garage or outdoors if possible. The doctor will use their discretion for cats that are skittish outdoors.

Reason for Euthanasia *

I, the undersigned, am at least 18 years old, and I am the owner / duly authorized agent for the owner of the animal described herein. I hereby consent to and order euthanasia (humane death) to be performed by Tranquil Crossings on this animal forever releasing said doctor, hospital, and all agents from any and all liability for performing said euthanasia. It is my desire to provide for my pet decent and humane after-death care, complying with all legal requirements of the area. I authorize the attending veterinarian to now dispose of the remains in accordance with hospital policy, releasing the hospital, doctor and agents from any and all liability for performing after-death care, with the following stipulations included: *

By signing this waiver, I am confirming that I have checked with my cities local ordinances regarding appropriate burial of a animal. I acknowledge that a Tranquil Crossings representative has advised me that at least 3-4 feet of earth on top of the body will prevent wildlife from encountering my pets body. I further acknowledge that I have been informed that if my pets body is incorrectly buried and wildlife consume any remains that they could then also succumb to the medications used during euthanasia. *

If the doctor arrives and I do not abide by said protocols I understand the doctor may leave the appointment without performing the euthanasia. I acknowledge that canceling an appointment with less than 24 hours notice may result in a non refundable $100 fee; cancelling after the doctor is already enroute for appointment will result in a $200 non refundable fee plus mileage if greater than 10 miles driven. *
